Consent To Treat Form:

Before we treat new patients, we would like you to please read the Holt Osteopaths Treatment Consent Form which simply means you have read and understood what the appointment will involve and give your permission for treatment.

You can either print this form off and sign before your appointment and bring it with you to the appointment, or we can provide you with a Treatment Consent Form to sign on your first appointment. Download Consent To Treat Form

If there are no appointments available in Holt, then please check the availability in Swaffham

Parking In Holt:

  • If you’re making an appointment for the Holt Practice, please be aware that there is no parking in the mews.
  • There is parking on the main road which is free and please park there to avoid any confrontation with the residents of Railway Cottages Mews.

Parking In Swaffham:

  • There is 2-hour free parking directly outside the practice (no need for a ticket).
  • If you park in the main square, then you’ll need to display a ticket.
  • Please remember the Swaffham Market is on Saturdays & we recommend parking for free in Theatre Street Parking (PE37 7HF).
